Plans are reviewed regularly and adjusted as your needs change — you’re always involved in the process.

Your healthcare worker will introduce themselves, review the care plan together, and take time to understand your home environment and preferences. The first visit is about building comfort and trust. If anything doesn’t feel right, your care coordinator is a phone call away.

We actively encourage feedback—it’s how we continue to improve. You can speak directly with your care coordinator anytime, as we’re available 24/7, 365 days a year. You can also contact your local branch, call our after-hours number provided in your welcome package, or submit feedback through the Family Portal.

The Family Portal gives you visibility into your loved one’s care schedule, visit details, and care team. You can access it anytime through our website or the app. It’s designed to give family members peace of mind, especially when you can’t be there in person.
